Commissioning Manager (Social and Healthcare Qualified)
As the role and the schemes develop there is the potential for line management responsibilities.
There is a responsibility for the post holder to build and maintain strategic provider relationships on a day-to-day basis Context of Work
The Commissioning Manager – Extra Care will report to the Strategic Commissioning Lead and will be responsible for the development and delivery of the commissioning strategy for Extra Care Housing, and for leading adult social care commissioning input in Housing policy and Planning policy on Retirement living and Sheltered Housing.
This will include undertaking needs assessment, designing and delivering pathways that turn commissioning intentions into positive outcomes for individuals, creating sustainable financial models within available resources and market engagement and partnerships.
There are four existing schemes plus a number of extra care developments underway, all at different stages, from early planning, to build stage, that will be a priority for oversight and setting of strategic direction.
The post holder will be expected to work closely with the developers, registered landlords, and support providers in any of the existing and new schemes in development. The post-holder may be required to line manage, lead and direct the work of Commissioning Officers in line with the organisational structure.
The Commissioning Team needs to ensure that it commissions with a strength based approach that takes into consideration both the strengths of individuals but also the strengths and assets of communities in whatever form they present.
Extra Care is an important part of the Council’s overall Adult Social Care and Housing strategy, with plans underway to increase provision through development of new schemes across the county.
Extra Care is a key enabler for maintaining independence, reducing reliance on more formal bedded care settings. Knowledge and experience of commissioning extra care / care and support with accommodation is key in order to inform and create proposals for:
- Finalising of Extra Care Strategy - Establishing and defining the Extra Care Service Model for Dorset
- Embedding processes and practices that enable accurate capture of extra care demand, diverting demand away from other residential / bed-based service models.
- Working with developers and lead care and support providers to input into building and service design
-Review existing schemes and models of care and propose options to re-commission and retain fit for purpose model.
- Further development of the opportunities place based care and support hubs can offer. - Consultation with existing tenants keeping them appraised of the options available and any possible changes to their support costs
