Main Duties:

  • Screen new CHC referrals- checklist and fast track.
  • Forwarded completed checklist to admin team for booking or feedback to the referrer if the checklist is incomplete.
  • Forwarded completed fast track (community) to ICB for ratification.
  • Complete clinical rationales/care plans for fast-track referrals and liaise with the referrer if information is missing or unclear.
  • Respond to clinical queries in our teams generic account- in absence of allocated clinician.
  • Document all interventions on EMIS and move referrals to correct pathway (once trained on Emis).
  • Complete death Datix - if case is not allocated to a clinician to follow up.

Requirements for the Continuing Healthcare Nurse:

  • Degree or equivalent in Nursing (RGN qualified) with active NMC Registration (Adult).
  • Recent nursing experience in the UK.
